Interesting question, Steven. I do believe that expression is used far more extensively in Britain than in the U.S. The wonderful Scottish singer Shirley Manson discusses her use of the word ‘twat’ and other swear words here.
I myself use the expression only occasionally, and exclusively in reference to men.

I’m Swiss, so by no means should you take my opinion seriously. I only EVER use this word for men, and I have no idea why. Apart from her really heavy swearing, I enjoyed yr reference to S.Manson 🙂
‘Twat’ was being used 65 years ago in the American Midwest to refer to the female pudenda. I think I laughed out loud the first time I heard it.
